| Question Number 1: Under the Freedom of Information Act 2000, I would like to request the following information relating to maternity services within your trust, for an academic research project. If some information is unavailable or cannot be provided within the cost limit, I would be grateful for any partial response you are able to provide. Does your maternity service routinely collect and record data relating to social risk, however this may be termed locally (e.g. vulnerability, complex social factors, social needs)? (yes/no) | |

| Question Number 2: If yes to Q1: approximately how many women using your maternity services were identified as having at least one social risk factor in the financial year 2024/25, where such data are routinely recorded? If an exact figure is not readily available, an estimate or the most recent available figure would be helpful. | |
| Answer To Question 2: The department have broken the data down into pre the launch of our Electronic Patient Record (BadgerNet) which started on the 1st March 2025. 1st January to 31st December 2024 (this data is pre our Electronic Patient Records) = 161 1st January 2025 to 28 February 2025 (this data is pre our Electronic Patient Records) = 5 1st March 2025 to 31st December 2025 (data from BadgerNet) = 169 | |

| Answer To Question 4: Social risk assessments are routinely completed at booking appointments. The Community Midwives review at the routine antenatal appointments. | |
