| Question Number 1: Freedom of Information Act 2000 request 鈥� paramedic-level patient transport from Salisbury to Dorset I am writing to request information under the Freedom of Information Act 2000. This request concerns arrangements made by SA国际传媒 for the return or onward transport of patients who live in the Dorset area from SA国际传媒 Hospital to Dorset hospitals and/or home addresses in Dorset, where the patient required transport involving paramedic-level care, enhanced care, high-dependency transport, urgent ambulance transport, or equivalent clinical support. For the purposes of this request, 鈥淒orset鈥� includes, but is not limited to, University Hospitals Dorset NHS Foundation Trust, Dorset County Hospital NHS Foundation Trust, and patient home addresses within Dorset. I am not seeking patient-identifiable information, clinical records, names of patients, addresses, NHS numbers, or any personal data. I am only seeking organisational, contractual, procedural, and activity information. If the Trust does not hold the requested information in exactly the form requested, please provide the closest equivalent recorded information held. Please provide the following information: 1. For the period 1 January 2024 to 24 April 2024, where a patient from Dorset required return or onward transport from SA国际传媒 Hospital to Dorset involving paramedic-level care, enhanced care, high-dependency transport, urgent ambulance transport, or equivalent clinical support: a. Which organisation(s), provider(s), booking hub(s), ambulance service(s), or commissioning body did SA国际传媒 contact or use to arrange such transport? b. Please provide any policy, procedure, guidance, flowchart, standard operating procedure, internal instruction, booking pathway, or written guidance used by staff when arranging such transport. | |
